Injury Waiver: I understand and agree that there is a possibility of injury in participating in dance classes, rehearsals, performances or related activities, and that I will not hold CCD, or any CCD faculty member, employee, board member or guest artist liable for any injury sustained or illness contracted while I/my child is participating at CCD. I exempt, release and indemnify CCD and its agents from any and all liability claims, demands, or causes of action whatsoever from any damage, loss or injury to the student, parent/guardian, family member, or personal
property which may arise out of or in connection with participation in any CCD activity.
While CCD has adopted comprehensive and aggressive protocols for cleaning, sanitization, and disinfecting of surfaces throughout its facility, I acknowledge the contagious nature of COVID-19 and other contagious diseases and viruses and voluntarily assume the risk that I and/or my children may be exposed to or infected by COVID-19 by attending and participating and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 and other contagious diseases and viruses may result from the actions, omissions, or negligence of myself and others, including, but not limited to, employees, independent contractors, volunteers, and program participants and their families.
I release CCD, its instructors, independent contractors and all associates from liability for harm, injury or death pertaining to COVID-19 and other contagious diseases and viruses. I acknowledge receipt of, and understand and agree to, CCD policies and procedures. I understand that my student must have this release form signed before attending class.
I also agree not to enter the CCD facility if I, my child, or anyone in our immediate family is displaying any symptoms of the COVID-19 virus as described in the organization’s health & safety protocols, and to either leave the facility or remove my child from the facility should any CCD faculty or staff member determine their physical condition represents potential exposure to the virus at any time you or they are inside the building. I understand I will not receive a refund or credit for classes or programs not attended.
This waiver covers the specific period from August 1, 2022 to July 31, 2023, and covers all programs, classes, rehearsals, performances, and all other activities inside the CCD facility at 3001 Industrial Ln. #12, Broomfield, Colorado, 80020 as well as all performance and community education venues. This waiver does nullify nor replace the CCD general waiver of liability required of all dancers.
